Making Healthcare Coverage Easier to Understand
Understanding insurance benefits can sometimes be confusing, but our team is here to make the process as simple as possible. Family Care of Chilhowie works with a variety of insurance plans and files claims on your behalf as a courtesy. We encourage every patient to become familiar with their individual insurance benefits, including deductibles, copays, and any services that may require prior authorization.

Accepted Insurance Plans
We participate with many major insurance providers and health plans. Because insurance networks and coverage change periodically, we recommend contacting your insurance carrier before your visit to confirm that our providers are participating in your specific plan.
If you are unsure about your coverage, please call our office. We will gladly assist you whenever possible.
What to Bring to Your Visit
To help us process your appointment efficiently, please bring:
Your current insurance card
A valid photo ID
Any referral or authorization required by your insurance plan
A list of your current medications
Updated contact information if anything has changed since your last visit
- Minimum of $50 per month
- Payments will be made until the total balance is satisfied
- Agreement will remain in good standing if payments are received by the due date of each month
- Payments will be overdue if more than (5) working days pass the due date
- A $25.00 fee will be charged if:
- 1. My account is five days PAST DUE
- 2. My credit card is DECLINED
- 3. My check is returned for INSUFFIENT FUNDS
- Any co-pays will be due at the time service (COPAYS MAY NOT BE CHARGED)

Written order from a primary care physician (PCP) to see a specialist

Approval from health plan before a service or medication is provided
Required to determine medical necessity
Who Are Self-Pay Patients?
Self-pay patients refer to those patients who pay the bill for medical services directly from their own pockets. They do so either because they lack insurance coverage or consciously opt not to use their health insurance.
